Protection of personal data
I, as a data subject, hereby grant my consent to the processing of personal data and information about the processing of personal data to OKF s.r.o, with registered office at Špitálka 8, 602 00 Brno, ID No. 25315650, registered in the Commercial Register maintained by the Regional Court in Brno, Section C, Insert 24909, as the controller of personal data (hereinafter referred to as the "Controller") in accordance with Article 6(1)(a) and Article 7 of Regulation (EU) 2016/679 of the European Parliament and of the Council dated 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (hereinafter referred to as the "GDPR"), I freely and unambiguously consent to the processing of my personal data (hereinafter referred to as "consent") under the conditions set out below:

The personal data of the data subject will be processed on the basis of the legitimate interest of the Controller for the purpose of direct marketing.

The Controller declares that the Personal Data will be disclosed to the relevant employees of the Controller that are obliged to maintain the confidentiality of the data, as well as the security measures the disclosure of which would compromise the security of the Personal Data.
The Controller declares that Personal Data will be transferred to PS Works s.r.o., with registered office at Jungmannova 1310/10, 697 00 Kyjov, ID No.: 28306201 (hereinafter referred to as the "Webmaster") for the above purposes, as this company provides web administration for the Controller. The obligation of confidentiality of Personal Data also applies to the respective employees of the processor.
The Controller also declares that it will not transfer Personal Data to third countries or any international organisation.

- the right to restriction of processing (the data subject has the right to have the Controller restrict processing in the cases set out in Article 18 of the GDPR)
- the right to data portability (the data subject has the right to obtain the Personal Data concerning him or her which he or she has provided to the Controller in a structured, commonly used and machine-readable format and to transmit those data to another controller, without hindrance from the Controller to whom the Personal Data have been provided, in the cases provided for in Article 20 of the GDPR)
- the right not to be subject to any decision based solely on automated processing (the data subject has the right not to be subject to any decision based solely on automated processing, including profiling which has legal effects concerning him or her or similarly significantly affects him or her pursuant to Article 22 of the GDPR)

Conclusion
The data subject hereby declares that he/she has been duly informed by the Controller about the processing of Personal Data in accordance with the provisions of Article 13 of the GDPR and that the above personal data is accurate and true and is provided to the Controller voluntarily.
